Basically the Eye of Cthulu, when it goes into it's 2nd form, can go out of bounds and either despawn, lose aggro on players and keep going, or it hits a kill plane. If it charges underground at all (so far for me 100%), you can NEVER finish the map.
On completion of a Chaos 4 map and selecting the 'continue' button to load the next map the game doesn't load anything and then a new selection of buttons appears asking us to either go to hub or return to title screen.

As stated above, occuring in most of games on new incu now. It's caused by one of the buffs gotten from crafting station (vile mushroom one). It makes poor eye die, before he can even transform or do anything.
in Malthius, I can jump from any edge of the map (where can I fall down and die) and use the monk's unique weapon from power surge to fly over the map and get to the spawn location of monsters and place some defenses in there.
